Requesting a Quote/Proposal

Submit the following:

  1. Name and address of the company
  2. Census (use document below)
  3. Current Plan Designs (SBCs)
  4. Two years of monthly employee counts and monthly claims
  5. Two years of high claimant information including diagnosis and prognosis
  6. Current renewal if available or prior year renewal
  7. Current list bill
  8. Please indicate the effective date requested by the client.
